Note: Záměr byl z důvodu reorganizace společnosti v dokumentaci předložen pod novým názvem "Pokračování hornické činnosti OKD, a. s., Dolu Karviná, v dobývacím prostoru závodu Lazy v období 2011 - 2015. " Z důvodu kontinuity procesu je však tento záměr i nadále veden pod názvem původním. Předmět posuzování zůstal shodný.
